Swing Bed

A Swing Bed Admission acts as a bridge between acute hospital care and discharge to another setting such as your home.

Some patients require extended care after an acute short stay and need more time to strengthen, heal, and/or adjust before returning to everyday life. The Swing Bed program provides the additional benefit of being able to move patients from acute care to skilled care for extended nursing and rehabilitative therapies to assist with this transition.

Our primary mission is to provide qualified caregivers and care managers that facilitate patients and their families/caregivers in achieving and maintaining an optimal level of independent functioning in order to safely transition to their home or other appropriate level of care.

Services and Team

Our Swing Bed program provides patients with an opportunity for short-term skilled nursing and rehabilitative therapy consisting of the following services:

  • 24 hour Skilled Nursing Care
  • Wound Care with Certified Wound Care RN
  • Wound Vac Management
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Respiratory Therapy
  • Transitional/Discharge Planning
  • IV/Antibiotic Infusions
  • Physician/Hospitalist
  • Care Manager
  • Dietician

Our Swing Bed team consists of highly skilled health care professionals who work collaboratively to develop an individualized care plan to assist each patient in achieving their optimal level of independent functioning. Our team provides education and requires patient participation in their care in order to achieve an optimal level of functioning. Our care team constantly communicates with the patient, family/caregivers and all disciplines involved, insuring each patient’s needs are met throughout their swing bed stay.

Coverage

Medicare coverage is limited to 100 days of skilled swing bed care. If you meet skilled criteria, Medicare will cover 100% of the first 20 days. You may be discharged before 20 days if your need for skilled services ceases to exist.

*Non-Medicare swing bed status is also available. Prior authorization is required for swing bed services that may be covered by private insurance . For more information about our Swing Bed Services, please call or email here or fax a referral here.

Qualifications

In order to qualify for our Swing Bed Program, a patient must meet the following requirements:

  • Be insured by Medicare Part A or other insurance with hospital based skilled care benefits
  • A “3 consecutive night” hospital stay as an “Acute Inpatient” within the last 30 days
  • An ongoing diagnosis or need requiring skilled care services related to the acute inpatient hospital stay that can only be provided in skilled nursing facility
