In order to protect the safety of our patients and staff due to the COVID-19 pandemic we are asking that patient requests for medical records, lab tests, radiology tests and billing records be obtained the following ways:
- Contact the Wayne County Hospital Health information Department at 606-340-3227 for verbal request for medical records.
- Fill out consent form of release of information on Wayne County Hospital (https://waynehospital.org/) website under Health Information Department-Medical Records Release and mail the request back to the hospital atAttention: Health Information Department, 166 Hospital Street, Monticello KY 42633
Medical records will be mailed, faxed, or emailed to patients upon request within 30 days. If you have any questions, please call the Health Information Department at Wayne County Hospital for further information.
